Building Inclusive Communities Through Individualized Care

We are a scaling Medicaid waiver provider that provides home and community-based services for adults with intellectual and/or developmental disabilities (I/DD).

Everyone Deserves the Right Support to Thrive

Community-based long-term supports and services are critical to assist many neurodivergent adults to achieve their daily goals, explore their dreams, live more independently, and harness their innate gifts to contribute their skills in society. These are individualized services, tailored to the needs and goals of each person and maximizing their choice. This can range from drop-in support to 24/7 supervision and high-acuity behavioral support.

“During her stay at SLS, Ava experienced exponential growth, personally and socially. Living at SLS, away from home, Ava’s self-worth increased. As she described it…‘after all, if other teenagers can go live in other towns and go to college, why can’t I?’ SLS staff expertly assisted Ava with her goals of leading an independent life. When a roadblock appeared with a certain task or goal, SLS staff created effective strategies to enhance achievement.”

SLS Community

Our Philosophy

  1. Live, Work & Play

SLS is committed to ensuring that all our Community Participants (CPs) can live, work, and play with self-selected, individualized, needed supports so that they may engage in rich and meaningful ways within their communities and flourish by meeting their goals.

  2. Your BloomPrint

Upon intake, a person-centered BloomPrint is carefully designed based on the evidence-based practice of gathering information through observation, interview, and assessment of skills, needs, and dreams.

  3. Goals That Fit You

This plan systematically considers the Community Participant’s interests, preferences, and goal suggestions across multiple domains of wellness, in accordance with their own definitions of success.

  4. ABA-Based Support

Our services are grounded in Applied Behavior Analysis methodology. We rely on ABA to use systematic, evidence-based methods to socially significant skills to reach individualized, objective, and measurable goals.
